You benefit from a financial support of the FSMP through one of its programs. To send us your reimbursement request, please fill the form below. FSMP cannot be considered as the beneficiary’s employer and is not bound by any liability or responsibility towards the beneficiary. By validating this form, the beneficiary accepts not to receive any other refund for the same expenses, except in case the expenses exceed the maximum amount paid by FSMP, and only for the excess.

The personal data registered in this form are strictly used for the reimbursement procedure and will not be used in any other way.
